SUMMARY OF JOB RESPONSIBILITIES:

Under the direction of the Director of Care and the Clinical Care Coordinator the RN coordinates the delivery of care to the Residents in accordance with the established standards, policies and procedures as set by Trillium Care Communities, the scope of practice of CRNBC and other legislated scopes of practice. In care teams, the registered nurse (RN) collaborates with the physician and other health care professionals as partners. RNs are crucial to the team’s achievement of better screening and health promotion, early detection, chronic disease management and enhanced access.

JOB DUTIES:

RNs work to their full scope of practice and this includes but may not be limited to:

  1. RN’s will be responsible for coordinating and supervising others who will deliver direct care, such as the LPN’s and Care Aides.
  2. Uses and adapts assessment tools and techniques based on the Residents unique needs.
  3. The registered nurse provides care and management for patients with a wide range of health issues. As an integral member of the health team, RNs provide care for complex Residents and those that require extra time and attention to develop a plan of care.
  4. Initiates and/or contributes to the development of a health plan in collaboration with the Resident, their family and other members of the interdisciplinary team.
  5. Encourages maximum independence and accountability for self-care.
  6. Determines the need for and initiates consultations with other services and health-care providers in a timely and supportive manner.
  7. Monitors, evaluates and adjusts the health plan based on effectiveness of interventions and/or changes in condition or environment, in collaboration with the patient and team members.
  8. Recognizes the need for and initiates effective interventions and treatments.
  9. The receiving and transcription of physician’s orders using a prescribed tool such as SBAR.
  10. Ensures that orders are correctly processed, recorded and acted on.
  11. Medication Administration to the full scope of practice as stated by the CRNBC.
  12. Provides emotional support and comfort to clients and their family. The pronouncement of expected death as per the RN scope of practice as set forth by CRNBC.
  13. Provides personal care such as assisting clients with bathing, dressing, care of skin and hair, assists with changing beds, toileting and ensuring client exercise programs are carried out as directed.
  14. Documents client care and progress as per CRNBC standards of documentation
  15. Making or adjusting resident assignments as required.
  16. Assists in maintaining a physical and psychosocial environment which meets the evolving care of the Residents.
  17. Ensures the proper use and care of equipment used in providing care to the Residents and promptly reports any malfunctions to the DOC or designate.
  18. Assists in maintaining adequate quantities of nursing care supplies on the floor.
  19. Reports to the DOC or designate all Incidents as outlined in the Community Care Facilities Licensing directive and completes the appropriate paperwork.
  20. To assist in the orientation of new staff and the mentorship of students.

  22. Assists with care of deceased. May pronounce expected death as per scope of practice per the CRNBC.

Implements and provides direction to volunteer programs.

 DUTIES:

 The duties of the Activity Coordinator include but are not limited to the following:

  1. Under the supervision of the Director of Care, researches, plans, implements and evaluates a variety of activities to assist in meeting the residents’ social, recreational, intellectual and physical needs.
  2. Plans for and schedules entertainment, volunteers, students, companions and special events including decorating the home.
  3. Escorts residents to special events and community outings.
  4. Consults with family and multidisciplinary team to determine the recreational needs of Residents and plans individual and group activities.
  5. Maintains equipment and work areas.
  6. Provides direction to coop students, volunteers and companions.
  7. Observes and reports changes in residents’ abilities, emotional well-being and social behaviours to the Resident Care team.
  8. Assesses residents’ and clients’ needs and develops, implements, monitors and evaluates comprehensive activation, day and outreach programs to meet those needs. Documents assessments on resident and client records. Notes any changes on the care plan related to activity program involvement.
  9. Identifies need for, recruits, screens, selects, trains, coordinates and monitors volunteers to support resident, client and outreach programs and terminates volunteers as necessary. Ensures appropriate documentation is maintained.
  10. Monitors expenditures and takes corrective action where necessary.
  11. Participates in resident care conferences.
  12. Maintains up to date records regarding resident activity attendance and use of additional services.
  13. Liaises with local clergy regarding services at Douglas Care.
  14. Maintains working knowledge of relevant legislation and regulations including Island Health directives, Public Guardian and Trustee Act, Employment Standards Act, Human Rights Legislation, WorkSafe BC Regulations, WHMIS, Douglas Care Policies and Procedures.
  15. Promotes, positive communication, cooperative working relationships and good public relations within and outside the facility.

Utilizing a variety of specialized computer software applications and working in a time sensitive, deadline driven environment, the Administrative Coordinator functions as a frontline member of the home and deals with residents, staff and public.

DUTIES

  1. Allocates and calls in relief staff to fill planned leaves, ongoing and short-call

vacancies and workload requests in accordance with established Staffing Services processes, and policies and procedures.

  1. Reference to collective agreements to ensure compliance.
  2. Performs a variety of staffing duties, such as:
  • Receiving and entering employee information and status, such as name, address, contact numbers, suitability for work, orientation and skills status,   hours worked, etc. into computerized scheduling system; gathering and entering for a 24 hour multi-department facility.
  • Posts positions in accordance with collective agreement and implements schedule changes
  • Scheduling for planned and unplanned absences;
  • Recording daily and relief staffing and schedule changes;
  • Investigating pay inquires;
  • Making timekeeping adjustments in computerized scheduling program;
  • Generating scheduling, staffing and timekeeping records and data/statistics for retention; and
  • Filing and maintaining records, such as phone logs, schedules, timecards, flow sheets and approved vacation and leave of absence forms.
  • Ensures employees meet annual and regular registrations with professional bodies as applicable, ensures checks are recorded in personnel files
  • Preparation of hire packages

SUMMARY OF JOB RESPONSIBILITIES:

Under the direction of the Director of Care and the Clinical Care Coordinator the RN coordinates the delivery of care to the Residents in accordance with the established standards, policies and procedures as set by Trillium Care Communities, the scope of practice of CRNBC and other legislated scopes of practice. In care teams, the registered nurse (RN) collaborates with the physician and other health care professionals as partners. RNs are crucial to the team’s achievement of better screening and health promotion, early detection, chronic disease management and enhanced access.

JOB DUTIES:

RNs work to their full scope of practice and this includes but may not be limited to:

  1. RN’s will be responsible for coordinating and supervising others who will deliver direct care, such as the LPN’s and Care Aides.
  2. Uses and adapts assessment tools and techniques based on the Residents unique needs.
  3. The registered nurse provides care and management for patients with a wide range of health issues. As an integral member of the health team, RNs provide care for complex Residents and those that require extra time and attention to develop a plan of care.
  4. Initiates and/or contributes to the development of a health plan in collaboration with the Resident, their family and other members of the interdisciplinary team.
  5. Encourages maximum independence and accountability for self-care.
  6. Determines the need for and initiates consultations with other services and health-care providers in a timely and supportive manner.
  7. Monitors, evaluates and adjusts the health plan based on effectiveness of interventions and/or changes in condition or environment, in collaboration with the patient and team members.
  8. Recognizes the need for and initiates effective interventions and treatments.
  9. The receiving and transcription of physician’s orders using a prescribed tool such as SBAR.
  10. Ensures that orders are correctly processed, recorded and acted on.
  11. Medication Administration to the full scope of practice as stated by the CRNBC.
  12. Provides emotional support and comfort to clients and their family. The pronouncement of expected death as per the RN scope of practice as set forth by CRNBC.
  13. Provides personal care such as assisting clients with bathing, dressing, care of skin and hair, assists with changing beds, toileting and ensuring client exercise programs are carried out as directed.
  14. Documents client care and progress as per CRNBC standards of documentation
  15. Making or adjusting resident assignments as required.
  16. Assists in maintaining a physical and psychosocial environment which meets the evolving care of the Residents.
  17. Ensures the proper use and care of equipment used in providing care to the Residents and promptly reports any malfunctions to the DOC or designate.
  18. Assists in maintaining adequate quantities of nursing care supplies on the floor.
  19. Reports to the DOC or designate all Incidents as outlined in the Community Care Facilities Licensing directive and completes the appropriate paperwork.
  20. To assist in the orientation of new staff and the mentorship of students.
  21. To perform other related duties as assigned by the Director of Care or the Clinical Care Coordinator.
  22. Reads and follows Acacia Ty Mawr’s policies and procedures.

  1. RN’s will be responsible for coordinating and supervising others who will deliver direct care, such as the LPN’s and Care Aides.
  2. Uses and adapts assessment tools and techniques based on the Residents unique needs.
  3. The registered nurse provides care and management for patients with a wide range of health issues. As an integral member of the health team, RNs provide care for complex Residents and those that require extra time and attention to develop a plan of care.
  4. Initiates and/or contributes to the development of a health plan in collaboration with the Resident, their family and other members of the interdisciplinary team.
  5. Encourages maximum independence and accountability for self-care.
  6. Determines the need for and initiates consultations with other services and health-care providers in a timely and supportive manner.
  7. Monitors, evaluates and adjusts the care plan based on effectiveness of interventions and/or changes in condition or environment, in collaboration with the resident and team members.
  8. Recognizes the need for and initiates effective interventions and treatments.
  9. The receiving and transcription of physician’s orders using a prescribed tool such as SBAR.
  10. Ensures that orders are correctly processed, recorded and acted on.
  11. Medication administration to the full scope of practice as stated by the CRNBC.
  12. Provides emotional support and comfort to clients and their family. The pronouncement of expected death as per the RN scope of practice as set forth by CRNBC.
  13. Provides personal care such as assisting clients with bathing, dressing, care of skin and hair, assists with changing beds, toileting and ensuring client exercise programs are carried out as directed.
  14. Documents client care and progress as per CRNBC standards of documentation
  15. Making or adjusting resident assignments as required.
  16. Assists in maintaining a physical and psychosocial environment which meets the evolving care of the Residents.
  17. Ensures the proper use and care of equipment used in providing care to the Residents and promptly reports any malfunctions to the DOC or designate.
  18. Assists in maintaining adequate quantities of nursing care supplies on the floor.
  19. Reports to the DOC or designate all Incidents as outlined in the Community Care Facilities Licensing and completes the appropriate paperwork.
  20. To assist in the orientation of new staff and the mentorship of students.
  21. To perform other related duties as assigned by the Director of Care
  22. Assists with care of deceased. May pronounce expected death as per scope of practice per the CRNBC.
  23. Reads and follows Bevan Village’s policies and procedures.

RESPONSIBLE TO:   Director of Resident Care, Registered Nurse,  Licensed Practical Nurse

QUALIFICATIONS:

  1.  Graduation from a recognized program for Care Aides
  2. Previous experience in a residential care setting preferred.
  3. Clearance to work with vulnerable adults as defined by the Ministry of Justice, Criminal Record Check
  4. Certification in WHMIS required
  5. Foodsafe and First Aid preferred.
  6. Registration with the BC Care Aide & Community Health Worker Registry as per legislation passed on June 30, 2010.

DUTIES:

  1. Under the direction of the nurse, carry out the day to day care of residents including hygiene, bathing, dressing, and feeding. Manage resident incontinence.
  2. Participate in assigned treatments as directed by supervisor.
  3. Report any unusual occurrences to the nurse including changes in resident condition or behaviour.
  4. To be aware of all resident activities and enable residents to attend the programs.
  5. To report to the Nurse/Director of Care /Maintenance team any malfunction or emergencies regarding any equipment.
  6. To complete laundry and meal service duties.
  7. To participate in any education programs offered and to attend any meetings or daily team conferences.
  8. To assist in the admission of new residents ensuring clothing and personal possessions are marked with the resident’s name.
  9. To assist in the orientation of new staff and the mentorship of students.
  10. To read and follow the Trillium Communities policies, procedures and Employee Responsibilities.
  11. To maintain resident confidentiality.
  12. To represent the facility in a positive manner both within the facility and in the community.
  13. To be a positive team member and work cooperatively with other team members in seeking to provide excellence in care and contributing to a positive work environment.
  14. To perform other related duties as assigned.
