This position requires a very special person willing to bring their expertise in Long Term Care.  The key responsibilities of this role are:

Under the direction of the Executive Director, you will assume responsibility for the care and well being of residents in the home.

  • Ongoing responsibility for making or adjusting resident assignments
  • Coordinating the use of staff, equipment and other resources
  • Interviewing and hiring for care team staff members
  • Assigning the responsibility for the care of residents to staff
  • Coordinate the care given by others in an area or unit.  The DOC is responsible for the active and on-going monitoring and follow-up of the work of others and accountable for the outcomes of the care given and can provide disciplinary action as required in collaboration with the Executive Director.
  • Identifying and recommending training/skill enhancement.
  • The DOC provides nursing care in accordance with administrative policies, physicians’ orders, established standards and nursing principles, and the  Community Care Facilities Residential Care Regulations.
  • Promotes a positive team spirit within the home.
  • Ensures up to date individual resident care plans are maintained through interdisciplinary care conferences and care planning.  Leads care  conferences.
  • Review and Admission process of residents from Pathways program, assist with the routine Discharge of residents and co-ordinate the       emergency transfer of residents to hospital.
  • Ensures that physician orders are correctly processed, recorded and followed through according to established policies for all staff.
  • Assists in maintaining a physical and psychosocial environment which meets the needs of the residents.
  • Plans assignments for care aides and other nursing staff – delegates accordingly to provide effective resident care.
  • Participates on facility/department committee(s), i.e. Nursing Practice, Quality Assurance and Resident Safety Committee, M.S.A.C, Labour       Management and Bargaining Committees.
  • Deals tactfully and courteously with residents, residents’ families, visitors and other staff members and responds to family complaints and concerns as required.
  • Assists the ED in training, supervising and evaluating departmental nursing personnel.  Takes an active leadership role in the department.  Able to investigate and discipline staff as required.
  • Maintains currency related to directives/memos/policies within facilities.
  • Leads activities related to the Infection Control and Wound Care Programs.
  • Participates in CQI responsibilities and leads CQI initiatives as required.
  • Is knowledgeable of the Resident Bill of Rights and endeavours to respect and promote it.  Ensures resident satisfaction and continuous quality improvement initiatives are promoted.
  • Ensures the proper care of equipment used in providing care to residents.
  • Attends and initiates in-service education programs and assists with nursing research and teaching where appropriate.
  • Participates in learning needs assessment.
  • Assists in maintaining adequate quantities of nursing care supplies on the unit.
  • Maintains a good attendance record according to Company policy.
  • Performs other duties as assigned by the ED.

 

CLINICAL DUTIES

  1. Wound Care Management – Assessment, Treatment and Monitoring of wounds.
  2. Administration of medications and functional assessments as required. Collaboration with physicians.
  3. Review and Process physician orders as required.  Clinical care planning, reviews and modifications. Assess resident’s needs on a regular basis and modifies individual Care Plan accordingly as required.
  4. Evaluate the effectiveness of care and inform healthcare team of significant changes.  Recommend and implement changes to the care plan.
  5. Monitor all aspects of nursing care to the residents.  Responsible for the organization and direction of R.N.’s, L.P.N.’s , &  H.C.A’s.
  6. Perform Pronouncement of Death as required.
  7. MDS/RAI – assessment of medical diagnosis and implement appropriate nursing interventions. The goals focus on achieving and maintaining optimal health, wellbeing, and quality of life. Filling out of the clinical data on R.A.I. as required.
  8. Risk assessments on admission as required.
  9. Functional Assessments – head to toe assessments for mobility, skin integrity, vital signs, pain management, range of motion as required.
  10. Medication Safety Committee – Monitoring and addressing med errors and following up with staff including disciplinary measures as required.
  11. Leads Multi-disciplinary Care Conferencing to evaluate the progress of the residents and modify care plans.

QUALIFICATIONS

  • Graduation from a recognized program for Registered Nurses or Registered Psychiatric Nurses.  Current practicing registration with the CRNBC.
  • Demonstrated leadership ability with comprehensive knowledge of nursing and health care practices and techniques as they relate to long term care.
  • Maturity and good physical and mental health, empathy, patience and tact are important qualities to possess.
  • Seeks out opportunities to maintain competency and continued learning.
  • Post RN/RPN education in Gerontology or three (3 – 5) years relevant experience.
