Responsibility

The Director of Care is responsible for the efficient and effective operation of the home, programs and the maintenance of safe and effective care for residents and clients to ensure that their needs are identified and met in a caring and compassionate manner. The Director of Care implements the policies and directives of the Organization within the vision, mission and values of Trillium Communities and ensures compliance with relevant legislation and regulations. The Director of Care maintains professional contacts with other organizations and agencies and liaises with the broader community.

Duties

  1. Plans, organizes and directs the daily operation of all resident care ensuring that a high standard of holistic, resident focused health care for the residents is maintained.
  2. Acts as the Facility Manager on the facility license and completes licensing reports, participates in investigations and inspections.
  3. Provides leadership and direction for the operations of all programs, services, and the facility and directly supervises the resident care including support services, maintenance and business office.
  4. Complies with all relevant legislation and regulations.
  5. Is responsible to meet the financial goals of the company. The Director of Care is accountable for the application of company financial policies and directions, and for the financial management of the facility. The Director of Care advises the Chief Operating Officer on all financial matters of the facility.
  6. Administers company and facility policies & procedures to ensure effective and efficient resident care.
  7. Provides direction and leadership to staff, including communication and interpretation of policies & procedures. Ensures the proper maintenance of all resident records in accordance with policy and relevant legislation.
  8. Ensures proper staffing levels are maintained and all staff are properly orientated and evaluated. Maintains regular performance appraisals of leadership team.
  9. Identifies areas educational needs of staff and provides and/or organizes in-services and workshops.
  10. Maintains complete, confidential employee records.
  11. Promote effective communication between staff groups/residents and staff and conducts problem-solving meetings, as required.
  12. Investigate all resident, family and staff complaints then take appropriate action.
  13. Provide Continuous Quality Improvement Initiatives through strategic planning.
  14. Ensures that all communications regarding the Resident’s status or needs are carried out appropriately – within the facility, to families or others.
  15. Participates in monitoring, auditing and follow-up related to all aspects of care.
  16. Monitors all aspects of nursing care to the residents with the assistance of the Clinical Care Coordinator. Responsible for the overall organization and direction of R.N.’s, L.P.N.’s, & H.C. A’s.
  17. Overall responsibility for the Admission process of residents. Ensures co-ordination of emergency transfer of residents to hospital meets the provision of safe care.
  18. Chairs Medication Safety Committee-Monitoring and addresses med errors and systems within facility.
  19. Work collaboratively with other members of the Health Care Team and other Programs within the facility.
  20. Participates and oversees the Resident Safety Committee and Resident Staff Committee. Leads the Leadership Team Committee.
  21. Organize, direct, co-ordinate and evaluate residential programs relating to the care of Residents.
  22. Administers a sound personnel program including effective procedures for recruitment, orientation, supervision, education, evaluation and discipline of adequate numbers of suitable and competent employees.
  23. Ensures that all public relations matters are handled according to Trillium Communities policy.
  24. Develops and maintains policies and procedures regarding the handling of concerns and complaints related to the services provided by Douglas Care Community. Investigates complaints and initiates appropriate action.
  25. Establishes and maintains effective working relationships with community organizations, government, funding and regulatory agencies.

Qualifications:

  • Current CRNBC registration
  • Bachelors Degree in a Nursing, Masters preferred
  • Successful completion of a recognized course relevant to Health Care Administration
  • Minimum of 5 years experience with proven competence in a management position in Health Care Administration, preferably in a Long Term Care facility
  • Demonstrated leadership and ability to communicate effectively with residents, families, volunteers, staff and external agencies.
  • Working knowledge of legislation, regulations and collective agreements and ability to work effectively in both unionized and non-union environments.
  • Working knowledge of computerized systems including common industry software.

  • Graduation from grade 10.

LPN's - Trillium Acacia Ty Mawr, Shawnigan Lake

RESPONSIBLE TO:   Director of Resident Care, Clinical Care Coordinator

QUALIFICATIONS:

  1. Graduation from a recognized program for Licensed Practical Nurses
  2. Current practice registration with CLPNBC without restrictions.

SUMMARY OF JOB RESPONSIBILITIES:

Under the direction of the Director of Care and the Clinical Care Coordinator the LPN coordinates the delivery of care to the Residents in accordance with the established standards, policies and procedures as set by Trillium Care Communities, the scope of practice of CLPNBC and other legislated scopes of practice.

JOB DUTIES:

LPN’s work to their full scope of practice and this includes but may not be limited to:

  1. The receiving and transcription of physician’s orders using a prescribed tool such as SBAR.
  2. Ensures that orders are correctly processed, recorded and acted on.
  3. Medication Administration to the full scope of practice as stated by the CLPNBC.
  4. Provides emotional support and comfort to clients and their family. The pronouncement of expected death as per the LPN scope of practice as set forth by CLPNBC.
  5. LPN’s will be responsible for coordinating and supervising others who will deliver direct care, such as the Care Aides.
  6. Provides personal care such as assisting clients with bathing, dressing, care of skin and hair, assists with changing beds, toileting and ensuring client exercise programs are carried out as directed.
  7. Documents client care and progress as per CLPNBC standards of documentation
  8. Participates as a member of the interdisciplinary team by assisting in the development, implementation, assessment and evaluation of client care plans.

  10. Assists in maintaining a physical and psychosocial environment which meets the evolving care of the Residents.
  11. Ensures the proper use and care of equipment used in providing care to the Residents and promptly reports any malfunctions to the DOC or designate.
  12. Assists in maintaining adequate quantities of nursing care supplies on the floor.
  13. Reports to the DOC or designate all Incidents as outlined in the Community Care Facilities Licensing directive and completes the appropriate paperwork.
  14. To assist in the orientation of new staff and the mentorship of students.
  15. To read and follow the Lodge’s policies and procedures.
  16. To perform other related duties as assigned by the Director of Care or the Clinical Care Coordinator
  17. Assists with care of deceased. May pronounce expected death as per scope of practice per the CLPNBC.
  18. Reads and follows Acacia Ty Mawr’s policies and procedures.

SKILLS:

  1. Ability to communicate effectively both verbally and in writing.
  2. Ability to deal with others effectively.
  3. Physical ability to carry out the duties of the position.
  4. Ability to organize work.
  5. Ability to operate related equipment
  6. Ability to work as a member of a multi-disciplinary team.
