  6. Provides personal care such as assisting clients with bathing, dressing, care of skin and hair, assists with changing beds, toileting and ensuring client exercise programs are carried out as directed.

  8. Participates as a member of the interdisciplinary team by assisting in the development, implementation, assessment and evaluation of client care plans.

  1. RN’s will be responsible for coordinating and supervising others who will deliver direct care, such as the LPN’s and Care Aides.
  2. Uses and adapts assessment tools and techniques based on the Residents unique needs.
  3. The registered nurse provides care and management for patients with a wide range of health issues. As an integral member of the health team, RNs provide care for complex Residents and those that require extra time and attention to develop a plan of care.

  5. Encourages maximum independence and accountability for self-care.

Under the direction of the SSM/DOC the Activity Coordinator develops, implements and evaluates a wide variety of therapeutic recreation programs which are consistent with the needs and interests of the Residents of St. Charles Manor Community in accordance with the established goals and policies. Implements and provides direction to volunteer programs.

 DUTIES:

 The duties of the Activity Coordinator include but are not limited to the following:

  1. Under the supervision of the Support Service Manager/Director of Care, researches, plans, implements and evaluates a variety of activities to assist in meeting the residents’ social, recreational, intellectual and physical needs.
  2. Plans for and schedules entertainment, volunteers, students, companions and special events including decorating the home.
  3. Escorts residents to special events and community outings.
  4. Consults with family and multidisciplinary team to determine the recreational needs of Residents and plans individual and group activities.
  5. Maintains equipment and work areas.
  6. Provides direction to coop students, volunteers and companions.
  7. Observes and reports changes in residents’ abilities, emotional well-being and social behaviours to the Resident Care team.
  8. Assesses residents’ and clients’ needs and develops, implements, monitors and evaluates comprehensive activation, day and outreach programs to meet those needs. Documents assessments on resident and client records. Notes any changes on the care plan related to activity program involvement.
  9. Identifies need for, recruits, screens, selects, trains, coordinates and monitors volunteers to support resident, client and outreach programs and terminates volunteers as necessary. Ensures appropriate documentation is maintained.
  10. Monitors expenditures and takes corrective action where necessary.
  11. Participates in resident care conferences.
  12. Maintains up to date records regarding resident activity attendance and use of additional services.
  13. Liaises with local clergy regarding services at St. Charles Manor.

 RESPONSIBILITY

 Reporting to the President, Trillium Communities, the Chief Operating Officer is responsible for leading operations and ensuring the provision of person-centered care among all facilities. Through effective management and engagement of the Facility Leaders, the Chief Operating Officer ensures clinical and quality standards are maintained in all Trillium Communities and that exemplary service is provided to all our residents.

The Chief Operating Officer ensures compliance that legislated regulations are met at all facilities and funding envelopes are managed appropriately.  He/She ensures standardized policy development across the company that meets legislated requirements.  The COO commissions new builds and ensures compliance with legislated regulations.

DUTIES

The primary roles and duties of the Chief Operating Officer include:

 Managing and overseeing facility leadership:

  • Conduct performance reviews
  • Prepare, implement and monitor leadership development plans
  • Work with leadership to develop annual facility plans
  • Participate in selection decisions for facility leadership team

Audit facility leadership and procedures at facility sites:
• Conducts audits within facilities to ensure facilities in compliance with legislation
• Audits facility leadership and receives operations reports from  individual facilities

Budgeting:
• Oversite of finance, development in collaboration with finance       department, and follow up on individual site performance

• Collaborates with Facility Leaders on financial targets and for sites/departments not meeting financial targets creates action plans and monitors progress of plan

Quality Improvement:
• Implement company wide and facility-based quality initiatives
• Support company-wide system rollouts for education and process improvements
• With the Facility Leader, ensure reporting of all Trillium and Health Authority Indicators; identify trends and issues and make recommendations for improvement

• Analyse data and collaborate with Facility Leader to identify areas for improvement, create action plan for improvement and monitor progress

• Support Facility Leader with timely submission of monthly, quarterly     and annual data to regulatory bodies (Health Authorities, CIHI, Ministry of Health)

Change Management:
• Plan and execute corporate changes that affect the region or the specific site. This may include change in contractor services, personnel, or processes.  Ensures that any change in policy or procedures at the sites receive proper authority and approval as defined under the legislation and quality improvement processes.
• Plan and execute Health Authority changes in processes or education that affects a region or a specific site.

Clinical Management Coaching and Contracted Care Service Provider Support:
• Communicate and assist in implementing care standards and policies for all contracted care providers including implementation of processes and procedures for collecting and reporting these standards on a regular basis.
• Assist in developing and implementing orientation guidelines for clinical management including contracted services.

Licensing and Health Authority Compliance:
• Acts as the licensee contact for facilities.  In coordination with the facility leaders, actively liaise with Community Care Facilities Licensing as it relates to facility management matters including investigations.

In coordination with the Facility Leaders, actively liaise with Health Authority Residential Care and representatives to ensure contractual obligations are met.

Critical Incident Support:
• Support Facility Leaders in their response to critical incidents: provide onsite support, investigation, reporting, liaise with various authorities, follow up on initiatives with staff residents and families.

Marketing and Communications:
Oversees marketing initiatives and ensures rental rates are aligned with market rental rates

• Ensure and marketing materials are aligned with organizational with vision and values.

• Collaborates with Facility Leaders on communications to families, community agencies and government agencies

• Acts as the organizational lead when communicating with media.

Human Resource Management:

Leads Employee Recruitment and Retention initiatives for the organization and collaborates with Facility Leaders regarding vacant positions

Supports hiring of qualified personnel for supervisory/management positions

Supports termination of personnel for all sites and departments

Coaches Facility Leaders with the management of employees who have performance concerns

QUALIFICATIONS:

Required:

  • Recognized education in areas of health administration, gerontology, and/or operational management;
  • Master’s Degree with specialization in gerontology, health care administration or  nursing required;
  • Minimum of 10 years of related experience with demonstrated leadership in senior positions in operations management and in a long-term health care setting;
  • Ability to communicate effectively; and
  • Good interpersonal skills and an ability to manage multiple critical timelines.

Desired:

  • Experience as a General Manager/Administrator at a long-term care, preferably multiple homes and a campus of care.

Additional Requirement:

  • Periodic travel to facilities within assigned portfolio.
  • Periodic extended stays near facility site locations.
  •  Valid BC Driving License

JOB DUTIES:

RNs work to their full scope of practice and this includes but may not be limited to:

  1. RN’s will be responsible for coordinating and supervising others who will deliver direct care, such as the LPN’s and Care Aides.
  2. Uses and adapts assessment tools and techniques based on the Residents unique needs.
  3. The registered nurse provides care and management for patients with a wide range of health issues. As an integral member of the health team, RNs provide care for complex Residents and those that require extra time and attention to develop a plan of care.
  4. Initiates and/or contributes to the development of a health plan in collaboration with the Resident, their family and other members of the interdisciplinary team.
  5. Encourages maximum independence and accountability for self-care.
  6. Determines the need for and initiates consultations with other services and health-care providers in a timely and supportive manner.
  7. Monitors, evaluates and adjusts the health plan based on effectiveness of interventions and/or changes in condition or environment, in collaboration with the patient and team members.
  8. Recognizes the need for and initiates effective interventions and treatments.

  3. Remove garbage and recycling from designated suites
  4. Perform light housekeeping in each suite including cleaning the bathroom, vacuuming carpets, sweeping and mopping floors and dusting.
  5. Respond to emergencies when directed
  6. Report needed supplies to the Director of Care
  7. If a resident is feeling unwell or a change in their wellbeing is noticed, report these concerns to the Director of Care.
  8. Communicate any resident concerns or maintenance concerns to the Director of Care
  9. Follow Daily Task Assignment Sheet for each day while working.
  10. Follow Infection Control Guidelines while working.

Other related duties as required:

  1.  Respond to requests from the RN/LPN on duty for bodily fluid clean up.
  2. Report to the Director of Care if a resident refuses housekeeping service
  3. Clean vacant suites for move in at the request of the Director of Care.
  4. Maintain confidentiality regarding all residents according to the Lodge policy.
